Consent to Treatment

By attending an appointment at our clinic, you acknowledge that you willingly give consent to our headache clinicians to perform any and all necessary examinations and treatment procedures in accordance with their professional training, qualifications and understanding of your injury. You understand that, during your treatment, different headache clinicians at Brisbane Headache and Migraine Clinic may be involved in your care.

You understand that you have the opportunity to discuss the nature and purpose of your consultation, with your headache clinician, before any treatment is performed, and that you may opt out of treatment at any time.

You acknowledge and accept the following:

  1. Treatment

1.1 Your consultation involves physical treatment, which may include “hands on” treatment, among other treatment options.
1.2 You have the right to choose the headache clinician that you wish to see, refuse treatment, or seek a second opinion at any time.
1.3 There are risks associated with treatment, including stiffness and soreness, soft tissue injury, neurological complications, cerebrovascular injuries, skin irritations, burns and other minor complications.
1.4 The chance of risk is lowered by a thorough assessment of your condition, and we take every precaution to minimise risk.
1.5 In many cases, other treatment options exist. If your condition is not musculoskeletal in nature, or the risk of your treatment is considered to be too high, your headache clinician may refer you on to other forms of care if they believe your condition is outside of their scope of practice.
1.6 There may be risk of remaining untreated. Remaining untreated can result in formation of adhesions, scar tissue and other degenerative changes. These changes can increase pain, which can result in chronic pain cycles, and can result in losses in range of motion and function. Delay in treatment can lead to further complications.

  1. Consent to Obtain and Release Information

2.1 To speed up processing and management of your injury or condition, you accept it may be necessary to communicate the details of your medical condition with treating practitioners, rehabilitation consultants, case managers, and solicitors that are involved in your case.
2.2 By signing these Terms and Conditions you give permission to Brisbane Headache and Migraine Clinic to exchange such information about your injury, condition or claim, as well as significant past medical history, with people deemed necessary by Brisbane Headache and Migraine Clinic.

  1. Exclusion of Liability

3.1 The director of the practice and any employees of this practice shall not be held liable for, nor accept any responsibility for any injury, loss or damage sustained by any person or persons resulting from treatments or procedures delivered in this practice or by our headache clinicians or in any way whatsoever which does not arise from any negligence of the practice director or the practice employees.

  1. Privacy and Contact

4.1 Brisbane Headache and Migraine Clinic will send you SMS or email confirmation for appointments

4.2 You may, from time to time, receive marketing or surveys from Brisbane Headache and Migraine Clinic. These are used to improve our services, and to offer our customers promotions and discounts.

4.3 You have the right to opt out of any SMS or email contact at any time.

4.4 Your details will never be shared with a third party except in cases where details are used to support you in your claim or injury, as outlined in clause 2. Consent to Obtain and Release Information.

  1. Payments and Billings

5.1 You are required to pay for the services provided, and any items provided, on the day of your consultation.

5.2 Payment can be made over the phone, or at the clinic where you attend your session. Other payments can be discussed with the admin staff, and may be available at their discretion.

5.3 There is no refund available on the purchase of any personal or healthcare items if you change your mind. If the item has a major problem we can either refund or exchange the item for you. If the problem is not major, we will repair it for you within a reasonable timeframe.

5.4 Our strict billing methods allow us to continue to provide high quality treatment for all of our patients, and are in no way designed to exclude or discriminate against any persons or groups of people.

  1. Late Arrivals and Missed Sessions

6.1 If you arrive late to your session, but within your appointment time frame, you may still receive treatment for a reduced time.

6.2 If you miss your appointment or arrive outside of your appointment time frame, you will be charged a cancellation fee of $50.

6.3 If you are able to organise a rescheduled appointment within the same business day, no cancellation fee will be charged. This is subject to availability of our clinicians.

6.4 You may cancel or reschedule an appointment so long as you provide us with a minimum of 24 hours’ notice prior to your appointment.

    1. Initial $50 Booking Reservation

7.1 For your initial appointment, we require a $50 booking fee to secure your booking.

7.2 If you need to reschedule or cancel this initial appointment, please notify us at least 24 hours in advance.

7.3 If you cancel before the 24-hour mark, you can only apply the $50 booking fee towards your rescheduled initial appointment.

7.4 However, if you fail to show up for your initial appointment or cancel within 24 hours of your scheduled time, the $50 booking fee is non-refundable.

7.5 The $50 initial appointment booking fee does not apply to initial appointments that will be managed by a 3rd party provider. This includes (but not limited to) WorkCover, NDIS plan-managed, CTP claims and DVA.

    1. Money Back Satisfaction Guarantee

Brisbane Headache and Migraine Clinic offers a guarantee on your first visit. During your first consultation, if the headache clinician cannot find the cause of your condition, then you will not have to pay for the consultation.

Your headache clinician will then refer you to another health professional who can assess and determine the likely cause of your condition. Our guarantee means that you have nothing to lose by booking your first consultation. This guarantee is made under the following terms and conditions.

8.1 This guarantee only applies for the first consultation and cannot be claimed for any sessions after the first visit.

8.2 A claim must be made immediately after the end of your first consultation and cannot be claimed for any session thereafter.

8.3 If we determine that we are unable to treat you, we will endeavour to refer you to another health professional.

8.4 If you paid for your initial consultation upfront, you will be reimbursed using your same payment method.
